FREQUENTLY ASKED QUESTIONS - FIRE AND RESCUE OPERATIONS DIVISION
I. Applicants must obtain a copy of the Fire and Emergency Report (SLKK) application form from the nearest Fire and Rescue Station and complete the application form along with the required documents;
II. The completed application form must be submitted to the Fire and Rescue Operations Division, State JBPM Headquarters accompanied by a receipt for payment of RM100;
III. Applicants must make a SLKK payment of RM100.00 at the Management Division, State JBPM and submit the payment receipt to the Fire and Rescue Operations Division, State JBPM;
IV. Payment must be made either by cash (cash money) / money order (money order) / postal order (postal order) / bank draft (bank draft). Payment by cheque will not be accepted.
V. The SLKK issuance process will take seven (7) working days from the submission of the completed form and documents;
VI. Applicants can collect a copy of the Fire and Emergency Report (SLKK) from the Fire and Rescue Operations Division in the states after seven (7) days of application.

I. Submit an application letter to the Malaysian Fire and Rescue Department (JBPM) at the State Level;
II. The State Level Secretariat (JBPM) will review for approval and support of the registration;
III. After receiving support and approval, they need to fill out and submit an application to the Malaysian Registration of Societies (ROS) online;
IV. Applications that have been approved by the Malaysian Registration of Societies (ROS) will receive an approval certificate online.
- Membership of this program is open to all levels of society aged 18 and above.
- Participation must be done using Form PB2001 which is available at all nearby Fire and Rescue Stations.
- The form must be made in 3 copies and forwarded to the Station Head for consideration.
- The Head of Station will review the application and then make a decision and support it.
- The Station Chief will submit an application to the State Fire and Rescue Director and obtain/issue a letter of appointment.
- Appointment letter from the State Director to the Station Chief.
- FIRE ELECTED FRIEND CERTIFICATE will be issued by the State Fire and Rescue Department to individuals who have been appointed.
